• Skin Cancer Treatment

  Skin cancer is one of the most common types of cancers. Luckily, with a little knowledge it can be identified and treated early. Types of Skin Cancer There are many terms to describe the different types of skin cancer that can occur. For ease of understanding, we can simplify all of these types of skin cancer into three categories: Basal Cell Carcinoma Squamous Cell Carcinoma Melanoma Basal Cell Carcinoma Basal cell carcinoma is the most common and the least aggressive of the three main categories of skin cancer. Basal cell carcinoma arises from the deepest part of the upper layer of the skin (epidermis). This type of skin cancer can be treated chemically (without surgery) or by standard surgical removal. Surgical removal only requires a very small amount (about 1 mm) of normal skin to achieve high cure rates. Basal cell carcinoma rarely spreads (metastasizes) beyond the actual tumor and therefore treatment does not need to be aggressive to achieve a cure. Squamous Cell Carcinoma Squamous cell carcinoma is the second most common type of skin cancer. This type of skin cancer also arises from the epidermal layer of the skin. Squamous cell carcinoma is more aggressive than basal cell carcinoma. It does have the possibility to spread beyond the borders of the tumor (metastasize). Treatment can still be done chemically or surgical removal. When possible, direct excision for single or few tumors is ideal. Margins of normal skin should be around 3mm on average. Melanoma Melanoma is the most aggressive form of skin cancer. The tumor arises from melanocytes or pigment producing skin cells. Melanoma is often black or purple in color but also can occur in a variety of other colors including pink and white. When caught early, melanoma is easily curable. Signs of identifying early melanoma tumors often include: Irregular borders Irregular color Irregular or asymmetric shape Greater than 5mm Surgical treatment for melanoma is recommended. The amount of normal skin around the tumor that should be excised depends on the depth of the melanoma noted on biopsy. Causes of Skin Cancer A combination of genetics and environmental exposure are the main causes of skin cancer. Of the environmental sources of skin cancer, sun exposure is by far the most common source overall. This is why sun block as well as routine personal skin exams are so important. • Breast Augmentation

  Breast Augmentation is the most common plastic surgery procedure in the country, followed only by rhinoplasty (nose job). Although breast augmentation may go by many different names, including breast implants, breast enhancement, and boob job, it all means the same thing: to increase the volume and improve the shape of the breast. There are two main ways to increase the size and volume of a breast: Breast augmentation with fat transfer is a process where fat is removed via liposuction (the third most common plastic surgery procedure) from areas where there is excess fat and then placed into the breasts. The main limitations of fat transfer breast augmentation is having enough fat and getting more than about one cup size increase in breast size in one fat transfer session. The benefit of fat transfer is of course not having a foreign implant and having completely natural breast feel. By far the most common type of breast augmentation is breast augmentation using an implant. Using an implant for breast augmentation allows the surgeon and patient to achieve nearly any size and shape of breast desired in one procedure. Although there is a ton of information related to breast implant augmentation, I feel there are really only four main points of discussion to determine the ideal breast augmentation plan for each patient. Breast implant size Breast implant type Under or over the muscle Incision used for placing the implant Breast implant size This is perhaps the most important part of the breast augmentation decision process. Obviously, if there is an ideal breast cup size or volume that a patient is imagining or desiring and the final result ends up being too small or too big, the patient will not be satisfied. To make sure this ideal volume is found, both a mathematical estimation is done as well as sizers used in the pre-operative meeting with the patient. The patient is instructed to bring in a sports bra that fits the cups size that they desire. Using a variety of sizer implants in the office setting allows the patient to fine tune their ideal implant size. Transaxillary/Endoscopic Breast Augmentation If you’re concerned about having visible scars after your breast augmentation, one possible solution is transaxillary breast augmentation, also known as endoscopic breast augmentation. Unlike standard breast augmentation, the transaxillary technique does not require any incisions around the areola or along the inframammary fold. Instead, Dr. Repta inserts the breast implants through small incisions located underneath each arm. This procedure offers the same natural-looking results as traditional breast augmentation, but the only scars will be hidden beneath the arms. • Breast Lift

  What is a breast lift. Simply put, a breast lift is any breast shaping procedure designed to lift and shape the breast into a more youthful and aesthetically pleasing appearance. Breast lifts are generally needed to correct breast ptosis. Breast ptosis is when the areola, breast tissue, or both have sagged below the border of the infra-mammary fold or breast fold. Purse-string breast lift A purse string breast lift is also known as a “donut lift” or a Benelli lift. These types of breast lifts use only a circular incision around the areola. Because of this, these types of breast lifts are sometimes also referred to as an areolaplasty. Because the incision used is only around the areola, only mild-to-moderate breast ptosis. A purse-string breast lift generally only elevates the areola higher and does not have the potential to significantly elevate and reshape the breast mound itself. Vertical breast lift A vertical breast lift is also known as a “lollipop lift”. It sometimes also goes by the name circum-vertical breast lift. This is perhaps the most commonly used breast lift as it is able to elevate the areola as well as the breast mound. When designed properly, a vertical breast lift can also “cone” the breast mound allowing the breast to be shaped from a flatter, longer appearance to a more compact and perkier appearance. For patients that have a lot of ptosis or a lot of loose breast skin, a vertical breast lift sometimes requires the vertical incision to curve toward the side of the breast preventing the incision from going down onto the abdomen skin. Full breast lift A full breast lift can result in a variety of different incisions. A full breast lift is designed to help women with significant loose skin or very ptotic breasts. When this type of breast lift result in a large horizontal incision as well in the breast fold it is often referred to an anchor breast lift because of the resemblance of the final incision line to the shape of an anchor. Although this is still routinely performed, I rarely use this method to accomplish a breast lift even in the most ptotic breasts. By redesigning the vertical breast lift, I am often able to perform significant breast lifts utilizing only a circular incision around the areola and a vertical incision that curves to the side. For women that have a lot of loose skin or tissue on the side of the breast and chest, I extend this “vertical” incision to allow me to remove the side breast and side chest tissue. This results in a “cleaning up” of the side of the breast/chest and often result in a much more aesthetically please breast with a well defined side-breast. I often refer to this breast lift as an extended breast lift. Common myths of a breast lift procedure: The areola is detached and re-attached. The areola is simply left attached to the breast tissue and the breast tissue is used to move the areola higher. Drains are needed for a breast lift. I do not use drains for breast lifts nor do I see the need for routine use of drains. Breast lifts result in loss of nipple sensation. Although nipple sensation loss is possible I have not encountered this in my practice. Breast feeding is not possible after a breast lift. Since the nipple and areola are left attached to the breast tissue, breast feeding should not be affected after a breast lift.
